This system is no longer accepting new consent responses
From September 2026, school vaccination consent is being managed through a new national NHS service called Manage Vaccinations in Schools (Mavis).
If you've received a link to this page from a previous school year, it is now out of date. No new consent or non-consent responses can be submitted here.
What you need to do
You do not need to do anything right now. Closer to your child's vaccination, you will receive a new email or text message with a link to the new online consent form. Please look out for this and respond when you get it — it's important to respond even if you do not want your child to be vaccinated.
Why this is changing?
The NHS is moving to a single national system used by immunisation teams across England, which makes it quicker and easier to record consent, track who has and hasn't responded, and manage vaccination clinics.
